A disease that infected 3.5 million people a year is down to ten cases

Guinea worm was everywhere in 1986. In 2025 there were ten human cases on the planet. There is no vaccine and no drug — it was pushed to the edge of extinction by filtering water and keeping a promise.
In 1986, guinea worm disease infected an estimated 3.5 million people a year across 21 countries.
In 2025, it infected ten.
Not ten thousand. Ten people, on the entire planet: four in Chad, four in Ethiopia, two in South Sudan. The Carter Center announced the figure on 30 January 2026. It is the lowest annual total ever recorded.

What makes this different from other health wins
There is no vaccine for guinea worm. There is no drug that cures it.
Guinea worm has been pushed to the edge of extinction without the tools that usually end a disease.
What ended it instead: teaching people to filter drinking water through cloth, treating ponds to kill the water fleas that carry the larvae, finding each case, and persuading the infected person not to enter the water while the worm is emerging — because that is the moment the worm releases larvae and the cycle restarts.
Cloth filters, pond treatment, surveillance and local cooperation, sustained for forty years.
If it is certified, guinea worm will be only the second human disease ever eradicated, after smallpox — and the first to be eradicated without a vaccine or a medicine.
The number that is not falling
An honest version of this story has to include the part that is going the wrong way.
Guinea worm is not only a human infection. It infects animals, principally dogs, and an infected dog entering a pond restarts the cycle just as effectively as an infected person.

Ten human cases is a remarkable number. But eradication is not certified on human cases alone — it is certified when transmission stops. As long as hundreds of dogs are infected in Cameroon and Chad, the last mile is not finished, and it is the mile that has taken the longest.
Adam Weiss, who directs the programme, put the human side of the count plainly: every case is a real person, known by name, enduring a disease we already know how to prevent.
When the total is ten, that is literally true. There is a list, and it has ten names on it.

In 2025 Cameroon recorded 445, Chad 147, Angola 70, Mali 17, South Sudan 3 and Ethiopia 1 — and the global animal total rose slightly even as four countries brought theirs down. Certification of eradication requires the transmission cycle to stop, not just human cases.
